Having a climbable trapdoor would allow for trapdoors at the top of ladders without the player getting stuck. This could apply to all trapdoors or just trapdoors with a new recipe. If it was a new item, the recipe could be the same as the grated trapdoor but rotated 90 degrees.

Standing on the top of the ladder, closing the trapdoor, and jumping isn't awful as means of getting around the current limitations of trapdoors above ladders, but I really do like this idea.

Honestly, might not even need a recipe. I don't see a reason why all trapdoors can't just count as ladders when open. It's a higher tier/cost item to build, so I don't think we're at risk of people spamming trap doors instead of ladders, aesthetically, it works, and logically, if there's a thick wooden trapdoor hanging open, I absolutely could grab onto it, and go up it with only slightly more inconvenience than going up a ladder.

So just make them always climbable in the open state!

If you place your trap door in such a way that it does not align with your ladder when opened, you will have an easier time leaving. Climbing all the way to the top of your ladder will put you next to the trap door whilst standing on top of the ladder, allowing you simply hop up the last block. Alternatively, once at the top of the ladder, you can first close the trap door, them jump up to slide through and on top of it.

Not as elegant as a trap door BEING a ladder, but at the very least it's a temporary bandaid until you find a real solution
