Even though I can start game, open to LAN, then open to online, my friend is saying they still can't join or find the game.  So.  What's the problem?  Do I actually have to buy a server or can I host a private game?

Welcome VikingHavel,

if you friend is on a different network (not in the same house) you most likely have to go through one or two extra steps, so that you friend can see your server: https://wiki.vintagestory.at/Setting_up_a_Multiplayer_Server#Ad-hoc_Server

Hi there, you need to do the following

Open port 42420 to your machine. If you cannot do that then you will need to use another method, Or you can just get hosting services. You will how ever had a better explanation with the link that Brady provided above.
