Torches disappearing from inventory

Having a constant issue with torches disappearing from my inventory. Also when i'm heating up an ingot having trouble with fire going out when i try to add an ingot.

Are you running any mods?

i've seen comments on things disappearing from inventory having to do with the certain mods

not that this is your issue, rain will extinguish the fire if no blocks are directly above the forge(?)
