Can/should I install this update from 1.21? I run a lot of mods

That's what I did. The second time this happened I died near to my base and it didn't even take a minute to go back. Stuff despawned. There was a spear in the ground, maybe that might have something to do with it? I'm not sure.

Had this happen to me twice now. My items despawn even though I went back to my deathpoint immediately. Am I missing something? Do I need to kill what killed me? A deer killed me. It is really frustrating to constantly lose your stuff. I play mostly vanilla with just animal cages, hud clocks, carry on, workbench expansion and visible high fertility. How can I at least cheat the items back?
