I'm trying to make something sorta like this with a mod, but I was just using the character creation menu to apply premade textures. I originally wanted the body paints from Orrukin because I really liked them (enough that I wanted a mod so that all character races from Vozzies' fantasy races could use them). I am an uber beginner at modding, but this seems like it would take more work. If I ever decide to try, I will update you! I do agree about piercings; I just do not know how they would look good since there are no nose models, but the fantasy races from Vozzies do have ears, so that could be as simple as making it possible to choose what ear you want and make new ear models with different earring configurations. But that would also mean making earrings in VSMC....
