Where are all the Elk Medallions?

I checked the handbook and the wiki, but there are several Elk Medallions that both have no recipe and also do not list what type of merchant will carry them. Are these special Lore related items that are only found in a certain location?

There are some in story locations

Aside from the craftable ones, there are medallions only sold by traders or found in ruins.
The trader ones are: 'full moon', 'scarab', 'cascade' and 'crescent moon'.
The ruin ones are all the others: 'forge', 'lake', 'star', 'night sky', 'crown', 'stiletto', 'strand' and 'cross'.
