Either "V" was off, or tried to logoff to avoid death and failed, etc. It would be very handy to somehow report the death coords to log file, or make an automatic screenshot with coords enabled. Ofc other players should not see this info, so e.g. chat is not good.

I ought to add an optional server component for Client+, but that is harder than it sounds (making it optional would be non-trivial) and I have way too many other things to do right now. I'll add it to my list though.
